These tools are forms for people with intellectual and developmental disabilities to fill out, with the help of family members or support persons if needed, and to share with health care clinicians to make health care visits or hospital stays more successful.

  • My Health Care Visit is useful for a regular office visit with a clinician.
  • My Hospital Form gives details to help ease a hospital stay.
  • My Coping Tool offers ideas about the patient’s own strategies for dealing with stressful situations. The forms ask about essential information, likes and dislikes, communication strategies, and needed accommodations, as well as offering some helpful ideas.

The Emergency Room Stabilization Protocol may be useful to print out and to take for ER visits, if possible, to share with staff there.
